Your visit to our office will most likely require the use of dilation drops. These drops often blur your vision for extended periods of time.
For your safety, we strongly urge you to bring an alternate driver or make other arrangements for your ride home.
Expect your visit to last one to two hours.
Please remember to bring:
• Glasses and/or contact lenses
• Health insurance cards
• Medicare/Medicaid cards
• Referral when appropriate
• Social Security card
• Driver's license
• A list of all medications you are taking
with dosage amounts
All patients that have insurance requiring a referral must have the referral in to our office 1 day prior to their appointment. If you do not have your referral at the time of your appointment, you will be re-scheduled.
Insurance company policies require that any co-pays must be made at the time of service.
